LIST OF DOCUMENTS TO BE ATTACHED WITH THE ADMISSION FORM

• Date of Birth Certificate (ORIGINAL)
• Proof of Residence (i.e. Voter’s Card/Driving License/Passport/Aadhaar Card)
• Proof of Sibling (wherever applicable (Real Brother/sister only) ) i.e. Fee Slip /Report Card.
• Proof of Alumni (wherever applicable) i.e. Passing Certificate (10th/12th)
• Aadhaar card of the child
• One passport size photograph of the child, father and mother.
• Medical fitness certificate duly signed by a licensed medical practitioner.

Note :

• It is mandatory to fill the form by Parents only and both of them must be present at the time of admission.
• Kindly attach Date of Birth (original) and self -attested photocopy of all the other documents as mentioned above.
• Aadhaar card of the child is mandatory.
• Fee Payment** has to be done by cheque only.
  **The fee may revise as per the approvals/directions given by DOE.
